RELEASE FROM LIABILITY

Voluntary Participation

I acknowledge that I voluntarily applied to participate in glass art work instruction (furnace, torch and/or kiln) and glass artwork (furnace, torch and/or kiln work areas) with supervision relative to my experience and capabilities, up to, and including no supervision if appropriate. Said activities are located at the premises of Mountain Robbins Art Studio, located at 20794 S.W. Hillsboro Hwy, Newberg, OR 97132. 

Assumption of Risk

I AM AWARE THAT CERAMIC ART WORK (FURNACE, TORCH, AND/OR KILN) MAY BE CONSIDERED A HAZARDOUS ACTIVITY.· I AM ALSO AWARE THAT PARTICIPATING IN THESE ACTIVITIES ON THE MOUNTAIN ROBBINS ART STUDIO PREMISES INVOLVES CERTAIN RISKS. I AM VOLUNTARILY PARTICIPATING IN THESE ACTIVITIES WITH KNOWLEDGE OF THE DANGER AND RISKS INVOLVED, HEREBY AGREE TO ACCEPT ANY AND ALL RISKS OF INJURY OR DEATH, AND VERIFY THIS STATEMENT BY PLACING MY INITIALS HERE:   _______

Release

As consideration for being permitted by the Mountain Robbins Art Studio to participate in these activities, and use of their facilities, I hereby agree that I, my assignees, heirs, distributees, insurance companies, guardians, and any/all legal representatives will not make a claim against, sue, or attach the property of J.W. Robbins (landowner), Wayne Robbins, Judie Mountain, Kim Van Antwerp, and/or Marianne Kasparian for injury and/or damage resulting from the negligence and/or other acts, howsoever caused, by any employee, agent, and/or contractor of Mountain Robbins Art Studio and/or any of its instructors, and/or staff, and/or volunteers as a result of my participation in ceramic art work (furnace, torch and/or kiln), any and all acts related to ceramic art work, and/or my presence on the Mountain Robbins Art Studio premises and use of their facilities.

I hereby release Mountain Robbins Art Studio and any of its instructors, staff, or volunteers from all actions, claims, or demands that I, my assignees, heirs, distributees, guardians, and legal representatives now have or may hereafter have for injury or damage resulting from my participation in ceramic art work (furnace, torch and/or kiln), any and all acts related to ceramic art work (furnace, torch and/or kiln), and/or my presence on the premises and use of the facilities. 

Knowing and Voluntary Execution

I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN ME AND THE MOUNTAIN ROBBINS ART STUDIO. I HAVE SIGNED IT OF MY OWN FREE WILL.
